Application for SSI

1. SOCIAL SECURITY CARD OR NUMBER

2. PROOF OF AGE 
(1). a public birth record recorded before age 5; or
(2). a religious birth record recorded before age 5; or
(3). other documents showing your age or date of birth.
If you already proved your age when you applied for Social Security benefits, you do not need to prove it again for SSI.

3. CITIZENSHIP OR ALIEN STATUS RECORD
(1). If you are a citizen, examples of documents you may need are:
A. birth certificate showing you were born in the United States; or
B. religious record of birth or baptism showing your place of birth in the United States; or
C. naturalization certificate; or
D. U.S. passport or passport card; or
E. certificate of citizenship.
(2). If you are an alien, examples of documents you may need are:
A. a current immigration document; e.g., an I-551 (Permanent Resident Card); or
B. I-94 (Arrival/Departure Record).
(3). If you are an alien who has served in the U.S. Armed Forces, you may need:
military discharge paper (form DD-214)

4. PROOF OF INCOME 
If you have income, you may need to provide the following:
(1). Earned Income - payroll stubs, or if self-employed, a tax return for the last tax year;
(2). Unearned Income - any records you have (for example, award letters, bank statements, court orders, receipts) showing how much you receive, how often, and the source of the payment; and
(3). Work Expenses - see information under SSI WORK INCENTIVES for more information

5. PROOF OF RESOURCES 
(1). bank statements for all checking and savings accounts
(2). deed or tax appraisal statement for all property you own besides the house you live in
(3). life or disability insurance policies
(4). burial contracts, burial plots, etc
(5). certificates of deposit, stocks, or bonds
(6). titles or registrations for vehicles like cars, trucks, motorcycles, boats, campers, etc

6. PROOF OF LIVING ARRANGEMENTS
(1). lease or rent receipt
(2). names, dates of births, medical assistance cards or Social Security numbers for all
(3). household members
(4). deed or property tax bill
(5). information about household costs, food, utilities, etc.

7. MEDICAL SOURCES (IF YOU ARE FILING AS BLIND OR DISABLED)
(1). If you are applying as a blind or disabled person, you need to give us:
(2). your medical reports, if you have them;
(3). names, addresses, and telephone numbers of doctors and other providers of medical
(4). services to you and the approximate dates you were treated;
(5). names of the prescription and non-prescription medications that you take.

8. WORK HISTORY
(1). job titles
(2). type of business
(3). names of employers
(4). dates worked
(5). hours worked per day and hours worked per week
(6). days worked per week, and rates of pay for work you did in the 15 years before you
(7). became unable to work because of your illnesses, injuries, or conditions
(8). description of job duties for the type of work you performed.

9. OTHER SOURCES
If you are applying as a disabled child (or on behalf of a disabled child), we need the names, addresses, and telephone numbers of people (teachers, caregivers) who can provide information about how the disabled child's medical condition affects his or her day-to-day activities. Also, if your child is part of an individualized education plan (IEP) at his or her school, it is helpful if you bring a copy of the plan with you.
